You are more than welcome to fill the forms out via the link on this page!

What to do: Click on the link for the form attached to the appropriate service and fill form out there. Be sure to sign the Cancellation Form attached to the top of your form and hit submit at the bottom of the page. Inform the spa coordinator that you have completed your forms upon arrival. *Please note that you must complete an intake form for EACH service that is being performed.
